THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
This Notice of Privacy Practices (the “Notice”) describes how Elara Health, as an affiliated covered entity composed of multiple distinct medical groups including but not limited to RSH Advisors, Rothschild Lawrence, and The 83rd Street Group, LLC, (collectively, “we” or “our”), may use and disclose your protected health information to carry out treatment, payment, or business operations and for other purposes permitted or required by law. “Protected health information” or “PHI” is information about you, including demographic information, that may identify you and that relates to your past, present, or future physical or mental health condition, treatment, or payment for health care services.
This Notice also describes your rights to access and control your PHI.
USES AND DISCLOSURES OF PROTECTED HEALTH INFORMATION
Your PHI may be used and disclosed by our health care providers, staff, and others outside our office who are involved in your care and treatment for the purpose of providing health care services to you, supporting business operations, obtaining payment for your care, and any other use authorized or required by law.
TREATMENT
We may use and disclose your PHI to provide, coordinate, or manage your health care and related services. This includes communication with third-party providers. For example, your PHI may be shared with another health care provider with whom you have an existing treatment relationship to ensure they have the information necessary to diagnose or treat you.
PAYMENT
We may use your PHI to bill or obtain payment for services. For example, we may use PHI to process payments for services provided to you.
HEALTH CARE OPERATIONS
We may use or disclose your PHI as needed to support the business activities of our office. These activities may include quality improvement, care coordination, offering information about treatment alternatives or other health-related benefits, developing or supporting computer systems, legal services, and conducting compliance activities such as audits and fraud, waste, and abuse investigations.
We may also de-identify and anonymize your information such that it is no longer considered PHI and contains no personal identifiers. We may modify or create derivative works using this de-identified information to enhance the services we provide. We may also use de-identified information for non-commercial purposes including analytics, research, educational publications, and case studies. We will never sell or commercially market your information.
USES AND DISCLOSURES THAT DO NOT REQUIRE YOUR AUTHORIZATION
We may use or disclose your PHI without authorization in the following circumstances:

USES AND DISCLOSURES THAT REQUIRE YOUR AUTHORIZATION
We will not use or disclose your PHI without your written authorization except as described in this Notice or as required by law. Should you provide authorization, you may revoke it in writing at any time.
You have the right to receive an accounting of certain disclosures of your PHI except those made for treatment, payment, or healthcare operations (unless maintained in an electronic health record) or disclosures made pursuant to an authorization.
You may request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.
REVISIONS TO THIS NOTICE
We reserve the right to revise this Notice and make the revised Notice effective for all PHI we maintain, including information already in our possession. Any significant changes will be posted on our Services. You have the right to object or withdraw as provided in this Notice.
BREACH OF HEALTH INFORMATION
We will notify you if a reportable breach of your unsecured PHI is discovered. Notification will occur no later than 60 days from discovery and will include a description of the breach, the PHI involved and contact information for questions.
